CHE 450G Lab Report Extension Request

Extensions are granted only for extremely extenuating circumstances such as those outlined in the course absence policy or when there have been serious problems with the experiment. You WILL NOT get an extension for "I have an exam that week", "I've got tickets to the Final Four" etc., so don't even try.

Fill out the requested information below and bring this form to the professor (not the TA's) in person. We will discuss your request and the available options.

All requests for extensions must be made at least TWO DAYS before the report is due.
